Becoming a Patient
How do I become a patient?
We are excited to welcome new patients to our practice! Our friendly administrative team is here to answer any questions you may have.
For new patients, we require the following documents:
- Prescription
- Demographic information
- Detailed office visit notes from your doctor
Please have your medical practice fax these documents to us at (401) 789-3190. Note that we cannot accept prescriptions directly from patients; they must be sent by your doctor's office.
